- Your answers about your symptoms and the amount you have set aside to invest

4. Text message and email communications
By submitting our application form you agree to receive SMS/text messages and emails from Rosanna Smith Health at the phone number and email address you provided. We use text messages only for your call: confirmations and reminders. Message frequency varies. Message and data rates may apply. Reply STOP at any time to stop receiving text messages, or HELP for help. You can unsubscribe from emails using the unsubscribe link in any email. Consent is not a condition of purchase.

No mobile information, including phone numbers and text message opt-in or consent data, will be shared with third parties or affiliates for marketing or promotional purposes.

What you tell us about your symptoms is used only to prepare for and hold your call. Our coaching and this website are educational and are not a substitute for medical advice, diagnosis or treatment.
